Supporting Families Through Case Management at Hill Country Community Ministries

At Hill Country Community Ministries (HCCM), we believe that true community support goes beyond immediate assistance — it’s about helping individuals and families build stability and regain hope for the future. That’s why we offer case management services designed to walk alongside our neighbors during times of need.

What is Case Management?

Case management at HCCM is more than a one-time service. It’s a personalized approach that connects individuals and families to the right resources, guidance, and long-term support. Each case manager works directly with clients to understand their unique situation, identify needs, and create a tailored plan for moving forward.

How We Help

Our case management program provides:

  • Assessment & Planning – We listen carefully to each client’s story, assessing immediate needs and creating a step-by-step plan for stability.
  • Resource Connection – We connect families with available programs, including housing assistance, food support, utility relief, employment services, and other community resources.
  • Ongoing Support – Our case managers provide continued encouragement, check-ins, and accountability, ensuring clients feel supported every step of the way.
